Personal Consumer Issues • Drilling into concrete foundation (for mounting screws)

(I'm good with drilling into wood and drywall, but have struggled to drill into concrete...)

Tried to mount something (device to be attached with screws) to our concrete foundation.

Used a good (corded) drill + masonry bit(s), but the problem is it's very hard to get the holes started at the correct, precise position. For drilling one hole this isn't such a big problem, but when trying to mount something that takes 4 screws at the ~corners, the misalignment of the holes is an issue.

Should I be using a hammer drill (I don't own one at present)?

Specialized bits?

Start with really small pilot holes (I started with a fairly big hole, ~1/4")

Is there an easy way to cover past hole mistakes - a putty/filler that is a good match for concrete color and/or slightly stainable if mine is a little darker/lighter than the default?

The thing I was trying to mount came with 4 plastic plugs + 4 metal screws. I think the idea is drill the hole, put in the plug, then drill the metal screw into the plug. I ~ruined at least one of these plugs on a mis-drilled hole. At the hardware store, I didn't see new equivalents of this - most stuff for going into concrete was all-metal and really big diameter (screws and/or other widgets). I can't use screws with really big heads, because for the thing I'm trying to mount, it's designed to mount on small-ish screw heads. Suggestions?
