I received a letter indicating that I have received paid medical treatment that maybe related to an accident, and I was asked to return a completed questionnaire. It is simply to fill out as the answer to the question whether the treatment was a result of an accident would be no. So no other questions to answer. I was asked to return the completed questionnaire within 30 days. I was given two ways to return it, by mail (using a postage paid return envelope) or by fax. I was hoping that I could do it by email as I would have proof that I have submitted it. My plan is to email myself a scanned copy of the completed questionnaire with a note of the date of submission. Any suggestion of a better idea?
