Personal Finance (Not Investing) • 1099-K From Paypal Sent to My Address

I received a 1099-K from Paypal for $76.95 with my address on it. However, instead of my name, it says "Fidelity National Information Services" as the Payee and shows the last four digits of a TIN that I don't recognize. It says "Payment Card and Third Party Network Transactions." I never received any payments from Paypal.

My actual Paypal account doesn't show anything with regards to sales and there is no tax form on my account either. I'm hesitant to contact Paypal and get my name associated with this. Anyone have any thoughts on this? Should do something with it or is the info so far off that it will never be associated with me (other than the address)? Thanks
