Personal Investments • How a mutual fund expense ratio is acutally charged?

I was going through my 457b MissionSquare Retirement portfolio and cannot understand from 01/01/24 through 11/12/24, why my Vanguard 500 Index fund (VFIAX, expense ratio of 0.04%) has charged me $418.96 in asset administration fees?

Based on my calculations, I should have only been charged ~$158. Every $10,000 at 0.04% is $4 in fees. Why am I being charged $260 more?

If my current balance in VFIAX is $395,677 my fees should be $158. (395677 X 0.04% = $158). Actually, the fees should be even less because my balance of $395,677 was less throughout this period. 

Is there a mistake that has been made or am I not understanding how the fee structure works? Or do you think they charging me their account management / record keeping fees and not recording it as a separate line item and just tacking it on to the ER fee being deducted?

Going through this years charged fees I only see fees being deducted for my actual mutual funds, nothing else.
